Want to drive on a school field trip? Please complete the training and turn in the below information to the school office

Turn Into Office:

• Driver’s License- copy front and back
• Insurance Declarations Page- copy from insurance company
• Volunteer Code of Conduct- signed copy
• Driver Information Sheet

Complete

• Child Protection Training (CAPP)
